Documents and Forms

UD forms are used by both prospective and current students to request financial aid from the University of Dallas. Once printed and completed, forms must be returned to the Office of Financial Aid.

Some students may be required to submit additional information after the FAFSA is received by the Office of Financial Aid in order to complete their financial aid application.

Note: All new borrowers accepting a Federal Direct loan(s), must complete a Direct Loan Master Promissory Note at StudentLoans.gov. If you have never completed Stafford Loan Entrance Counseling, you must also do so online StudentLoans.gov.
