All students borrowing a Federal Direct Subsidized or Unsubsidized loan(s) for the first time must complete both (1) Entrance Counseling and (2) Master Promissory Note (MPN) for the Subsidized/Unsubsidized loan(s).
- Both items must be completed here: www.studentloans.gov. This link will take you to the Department of Education Direct Loan website.
- You must use your (student) FAFSA PIN to sign into this site and complete the Entrance Counseling and MPN.
Please do not send UD your MPN or the confirmation page from your Entrance Counseling session. We pull daily reports to determine who has completed each item. | | |

Note: The Perkins MPN and Entrance Counseling may not be faxed or emailed; we must have the original signatures on each item. Please drop the form by, or mail it to, the Office of Financial Aid.
For students selected for Verification:
The federal government randomly selects FAFSAs to be verified by the financial aid department. A financial aid counselor must confirm that the information on the FAFSA is correct before the student may receive any financial aid.
When completing your FAFSA online, you have the option of automatically retrieving your income and tax data from the Internal Revenue Service and having it transferred to your FAFSA. The IRS Data Retrieval Tool is the preferred method to complete the FAFSA. The IRS Data Retrieval Tool is the fastest and easiest option and will satisfy the tax transcript requirement.
If you are required to request a tax transcript from the IRS (please see the Verification Worksheet), you may do one of the following:
- Paper request- IRS Form 4506T-EZ (recommended). This option will allow you to have the tax return sent directly to UD from the IRS. If the tax transcript is not sent directly to UD from the IRS, you must sign the transcript before sending it to our office.
- Call 1-800-908-9946. Please provide our fax number 972-721-5074 as the destination.
- Go online at www.irs.gov.
If you are not a US citizen or Permanent Resident, but you are classified as a Texas resident and therefore may be eligible to receive an in-state grant, please complete the 2012-2013 TASFA application. The Office of Financial Aid will review your TASFA application to determine your eligibility for state funds. This is for undergraduates only. | | |
