Steps to be taken in the Event of a Workplace Accident/Injury
Step 1 - Report the Incident
Any accidents, injuries or emergencies that occur on campus should immediately be reported to Campus Safety at X2911 (972) 554-2911. The Campus Safety Office is open 24 hours a day, seven days a week with officers on duty at all times. Once Campus Safety receives your call they will immediately respond with the appropriate action. In cases of accidents, injuries or emergencies, Campus Safety will dispatch an officer to the scene immediately to assist and to take further action if necessary. In the case that there is an injury, the Campus Safety Officer will first determine if the injured party requires medical attention up to and including emergency transport. Once the injured party has been taken care of, the Officer will then fill out an Incident Report (if applicable) and forward a copy of this to all appropriate parties.
Step 2 - For Employees When Medical Attention is Required
All active employees (this includes student workers) are covered by the University's workers' compensation insurance policy through The Zenith. Under the terms of this policy, covered employees are required to pick a Treating Doctor from the Zenith Health Care Network. For more information on the Zenith Health Care Network please click:

If you need emergency care you do not have to go through a ZHCN Treating Doctor, you should go to the nearest emergency room.

or you can call them at (800) 841-3987. For additional assistance in locating a Treating Doctor you can contact the Office of Human Resources at (972) 721-5382.
The University's doctor is not a Treating Doctor for workers' compensation injuries. This applies to student workers as well.
Step 3 - First Report of Injury
Employees who become injured on the job should report the incident to Human Resources as soon as possible at (972) 721-5382.
A First Report of Injury must be filed by UD with Zenith within eight days from the date the employee notifies the HR office of an injury that results in more than one day of lost time or of an occupational disease.
The injured employee is also required to sign and return to the HR office the ZHCN Workers' Compensation Network Acknowledgement

Attention: Supervisors
The following forms should be completed by the supervisor in the event there is a workplace injury. These forms should be completed and sent on to the Office of Human Resources immediately after the incident has occurred.

- This form should be filled out first in order to gather facts for the First Report of Injury form.

– This form should be filled out by the supervisor to the best of his/her ability and

University of Dallas Automatic External Defibrillator (AED) Program
The University of Dallas utilizes the ZOLL AED Plus. AEDs are permanently located in the Campus Safety Office in Haggar, on the 1st floor of the Gym, just inside the front entrance to the GSM building and on the wall outside the public restrooms in the Church of the Incarnation. An AED is kept in the CSO patrol car at all times. In addition, the Athletics Department has two AED units that are designated to be on site at all athletic events.
