New Hire Documents
Below is a list of documents required for student employees of the University. Documents marked with an asterisk are required.

Student Employee Information and Agreement*
Includes information about the student as pertaining to his or her employment and agreements (receipt of handbook, confidentiality, and general statement).

W-4* 
This form is the first step that the employer uses to send taxes to the federal government on your behalf. Please follow the worksheet carefully or use the Internal Revenue Services' website to assist you in deciding how many deductions to take if any at all. The IRS has an online deduction calculator available at http://www.irs.gov/individuals/article/0,,id=96196,00.html.

I-9*
The I-9 Form is a requirement for every employee on campus, including faculty, staff, and students. It is required by the national government with the intention of preventing illegal aliens from working. In order to remain employed on campus, and anywhere in the United States, you are legally required to provide original documents by the third day of your employment. If you are unable to provide the necessary documents, you will be asked to stop working until you can.

Payroll Memo & Deduction Authorization*
The payroll memo is required for students in order to electronically send the W-2 in January to the student's desired email address.

Even though as a student employee you might not receive any equipment, keys, etc., the deduction authorization is required by every employee on campus.

Email for Direct Deposit & Direct Deposit Form
Student employees are not required to participate in direct deposit. However, please keep in mind that paydays are 1.5 weeks after the last day of the pay period. This means that at the end of each semester you most likely will not be on campus to pick up your checks. For students who go to Rome, direct deposit is required.

If you would like to participate in direct deposit, complete both forms and attach a voided check to the direct deposit form.

Worker's Compensation Insurance*
Worker's Compensation Insurance does not supplant your regular medical insurance. This coverage is only available to employees for at-work injuries. Please read carefully. The last page is the only page that must be returned. Pages 11 and 12 are to be completed by your primary physician if you would rather that physician take care of you than a physician in the Zenith Health Care Network. These two pages are not required.
Student Employee Handbook
The Student Employee Handbook is applicable to all undergraduate student employees as well as Braniff and Graduate School of Management graduate assistants.
