This health improvement program is completely confidential. At no point will any information be shared with CARES or our medical insurance provider.
You recently received a packet of materials about The Prevention Plan. Be sure to review these materials, which include a brochure explaining the benefits of The Prevention Plan and a Quick Start Guide with detailed instructions about how to register and earn a $150 gift card to the retailer or restaurant of your choice!
Complete the First 3 Steps by March 31 & Step 4 by May 31 to Earn Your $150 Gift Card:
- Employer ID:0029-CARES-2010
- Individual ID: Use the ID number found on the front of your medical insurance card. This ID starts with “CAP” and is followed by 9 numbers. The Prevention Plan is and will always be COMPLETELY independent from your health plan, but this number is being used for your convenience. Employees must add an E to the end of the identification number, and spouses must add an S. Example: CAP123456789E and CAP123456789S (no space or dash should be used)
2) Complete your online Health Risk Appraisal
- This confidential questionnaire about your health will take 15-20 minutes.
3) Schedule and complete a free at-work blood test
- At-work blood tests will be available at the University of Dallas campus on February 14 & 15.
- If you can’t attend the test on campus, you can complete your test at any convenient LabCorp location.
4) Complete 2 Action Programs by May 31, 2012
- These educational tutorials are available in a wide variety of health topics, including Back Pain, Healthy Eating, Diabetes Prevention and Stress Management. Choose the topics that are recommended to you based on your health risks and learn how to better manage your health. Your personal Health Coach is available anytime Monday through Friday, 7 am to 10 pm to help you along the way.
