This page is primarily intended as a resource page for staff reporting to the Office of Student Life.
This form is intended for the use of any individual who witnessed an incident involving an University of Dallas student or has any concern(s) for an individual or group of students. Incidents include any and all violations of the Student Handbook. Concerns range from missed classes, overly stressed about school work, relationship problems or domestic violence, family issues (sickness, alcoholism, death, job loss), alcohol or drug abuse, repeated reckless conduct, and signs of depression or psychological issues.
Resident Assistants have access to the current floor plans via Google Docs. If you have issues logging in, please contact Betty Perretta in the Office of Student Life.

Any computer, email or internet access issues may be reported to firstname.lastname@example.org from a working computer. At a later date a central phone number will also be available.
RESIDENT ASSISTANT EVENT FORMS
This form must be submitted a minimum of 48 hours in advance of the event.
This evaluation form must be submitted within three (3) days after the event.
