Fall 2008 New Students
After confirming** enrollment with the Admissions Office, all incoming freshmen and transfer undergraduate students must complete the following forms for the Office of Student Life. Some forms pertain to the basic necessities of life while other forms assist with a smoother transition to college life. This requested information is non-academic. All academic questions should be directed to the Dean of Constantine College.
(**You are defined as a 'confirmed' student when you have sent in the required $400 enrollment deposit along with the 'Enrollment Decision Form'. This form is sent to you along with your acceptance letter from the University of Dallas Admissions Office. Please do not complete any of the forms below unless you fall into this category.)
- Medical History Form. Due to signature requirements this form is downloadable in PDF format. You will need to schedule a doctor's appointment to complete this form. Upon completion of the entire form, please mail to:

Failure to submit this form in a timely manner will result in a registration hold for your second semester at UD.
- Student Health Insurance Information. At the time of class registration, all full-time undergraduates are automatically enrolled in and charged for the student insurance program. However, the annual health insurance cost may be waived upon proper submission of a waiver form directly to the insurance company by the September 20, 2008 deadline date. (February 7, 2009 is the deadline for new spring enrolless to waive the pro-rated annual charge.)
Students may waive participation in the Basic Sickness Benefits by submitting the online waiver form by the above deadline date. As soon as the online waiver form web page is available, a direct link to the form will be placed here for your convenience. Check back periodically for the link.
Absence of a waiver form by the deadline date implies your acceptance of the program and related charges.
Special Note to International students attending on an F-1 visa: International Students must be enrolled in the university policy for International Students. Further information is available from the International Student Advisor, Marilyn White, 972-721-5059 or by email here.
- Due to the mandatory residency requirement students must complete EITHER a Housing Contract -OR- a New Student Commuter Form. All new students will automatically be billed standard room and board charges. Commuters' charges will be reversed upon receipt of a valid Commuter Form.
Residents will not be specifically assigned and matched with a roommate until...
.....receipt of the $150 housing deposit
.....completion and receipt of housing contract and
.....completion of online MBTI personality test (info below).
The $150 Housing Deposit may be submitted via a check or with a Credit Card Authorization Form. Either may be mailed to:

- MBTI Online Survey for on-campus RESIDENTS only. This is a tool to assist the Office of Student Life in the roommate matching process for new students. This form must be completed by the STUDENT and not the PARENT.
