This form is intended for the use of any individual who witnessed an incident involving an University of Dallas student or has any concern(s) for an individual or group of students. Incidents include any and all violations of the Student Handbook. Concerns range from missed classes, overly stressed about school work, relationship problems or domestic violence, family issues (sickness, alcoholism, death, job loss), alcohol or drug abuse, repeated reckless conduct, and signs of depression or psychological issues.
